Before you start

One item issue per submission

Please submit one claim for one item or device issue or incident.

Have these ready:

  • Policy number
  • Device make/model and IMEI or serial number
  • Incident date, time, location, and what happened
  • Proof of ownership or value

You may also need:

  • Photos of the damaged item or device
  • Crime reference or police report for theft
  • For lost or stolen mobile phones: network blacklisting/SIM bar confirmation and 30-day usage report

Please upload the documents that apply to your claim when you submit it. For provider or retailer documents, send the original file where possible, such as a PDF or email attachment. Screenshots are not accepted.
