One item issue per submission
Please submit one claim for one item or device issue or incident.
Have these ready:
- Policy number
- Device make/model and IMEI or serial number
- Incident date, time, location, and what happened
- Proof of ownership or value
You may also need:
- Photos of the damaged item or device
- Crime reference or police report for theft
- For lost or stolen mobile phones: network blacklisting/SIM bar confirmation and 30-day usage report
Please upload the documents that apply to your claim when you submit it. For provider or retailer documents, send the original file where possible, such as a PDF or email attachment. Screenshots are not accepted.
